Water Sports Neck Injury: What to Know If You're a South Florida Water Athlete

Living near the ocean, the Intracoastal, and South Florida's many waterways means water sports are part of life for a lot of our patients. Paddleboarding, surfing, kite surfing, wakeboarding, jet skiing — these activities are incredible, but they all carry real risks for neck injury.

Understanding what can happen — and what to do about it — keeps you on the water longer.

How Water Sports Injure the Neck

Paddleboarding and kayaking — These require sustained neck extension to look forward while the upper body is in a forward-flexed position. Over time, the muscles of the posterior neck fatigue, and the cervical facet joints and discs experience increased compressive load. Hours on a paddleboard without proper body mechanics is a fast track to neck stiffness and pain.

Surfing — The "pop-up" and sustained prone paddling position (neck extended for extended periods) are extremely stressful on the cervical spine. Cervical muscle strain and facet joint irritation are common, especially in beginner surfers and those who paddle long distances. Wipeouts can cause acute whiplash-type injuries.

Wakeboarding and water skiing — Falls at speed create significant forces on the spine. Neck injuries from sudden jerking or impact with the water surface can range from muscle strain to cervical disc herniation.

Kite surfing — Spectacular fun, significant injury risk. Hard falls and being dragged can cause acute cervical trauma. The sport also requires sustained overhead arm position that can impinge on shoulder and thoracic structures.

Symptoms That Warrant Attention

  • Neck pain that doesn't resolve within a few days of your last session
  • Stiffness or reduced range of motion (difficulty looking over your shoulder)
  • Headaches starting at the base of the skull
  • Numbness, tingling, or weakness in the arm or hand
  • Pain that worsens with continued activity

Arm symptoms (numbness/tingling) are a red flag for nerve involvement and should be evaluated promptly.

Treatment Approach
