Does Chiropractic Hurt? What to Really Expect from an Adjustment

The popping sounds you hear in videos. The idea of someone pushing on your spine. The general unfamiliarity of the whole thing. These are reasons a lot of people hesitate before their first chiropractic appointment — and they're completely understandable.

Here's the honest truth about what chiropractic treatment actually feels like.

The "Pop" Is Not What You Think

The cracking or popping sound associated with spinal adjustments is called a cavitation. When a joint is moved quickly through its range of motion, a small gas bubble (mostly carbon dioxide) releases from the joint fluid. That's the sound.

It is not:

  • Bones cracking
  • Joints being forced out of or into place
  • A sign that something went wrong
  • Painful (for most people)

The sound is incidental to the treatment. Some adjustments produce it; others don't. Whether you hear it has no bearing on how effective the treatment is.

Does the Adjustment Itself Hurt?

For most patients in most situations — no. The adjustment itself is quick and controlled. Patients often describe it as:

  • A feeling of pressure followed by release
  • Immediate relief or lightness in the area treated
  • A mild stretch
  • For some: a brief moment of discomfort that passes instantly

When you're in significant pain, any movement in the affected area can feel sensitive. Your chiropractor will position you carefully to minimize this and will always explain what they're about to do before they do it.

What About After the Adjustment?

The most common post-treatment experience is mild soreness in the treated area for 24–48 hours — similar to the muscle soreness you feel after a workout when your body isn't used to the activity. This is normal and is your body responding to the change in joint position and movement.

Many patients also notice:

  • Immediate reduction in pain
  • Improved range of motion
  • A feeling of being "looser" or lighter
  • Better sleep that night

First visits often produce the most noticeable change — both in terms of response (sometimes temporary soreness) and relief.

What If I'm Very Sensitive or Have Osteoporosis?

We always adapt our approach to the patient. If you have osteoporosis, recent fractures, significant joint degeneration, or you're simply anxious about forceful techniques, there are many alternative approaches that achieve the same goals gently:

  • Drop-table technique — uses a special table segment that "drops" with the adjustment force, making it feel very mild
  • Activator method — a small handheld instrument applies precise force without manual thrusting
  • Mobilization — gentle, rhythmic joint movement rather than quick manipulation
  • Soft tissue therapy — working the muscles and fascia without direct joint treatment

You should never feel like you have no options or that you have to endure something uncomfortable.
