Class IV Laser Therapy: Definition and Clinical Context

What Is Class IV Laser Therapy?

Class IV laser therapy is a therapeutic treatment that uses high-powered laser light — operating above 500 milliwatts of continuous power — to deliver photons deep into biological tissue, triggering a cascade of anti-inflammatory and tissue-regenerative effects. It is formally classified as photobiomodulation therapy (PBMT).

The Roman numeral "IV" refers to the laser's classification under FDA safety guidelines. Class I through III lasers are too low-powered to achieve meaningful therapeutic penetration. Class IV lasers deliver sufficient energy to reach tissue depths of 5–10 centimeters — enough to treat deep joint structures, spinal discs, peripheral nerves, and dense muscle tissue.

The Photobiomodulation Mechanism

When Class IV laser photons (typically in the 800–1000 nanometer near-infrared spectrum) penetrate tissue, they are absorbed by photoreceptors in the mitochondria — specifically cytochrome c oxidase, a key enzyme in cellular energy production. This absorption triggers several biological changes:

  • Increased ATP production: Cells produce more adenosine triphosphate (ATP) — the primary energy currency of the cell — which accelerates repair and metabolic activity
  • Reduced inflammatory cytokines: Class IV laser suppresses pro-inflammatory chemical signals (including prostaglandins and interleukin-1), reducing local inflammation
  • Improved microcirculation: Laser therapy dilates blood vessels and improves local blood flow, delivering oxygen and nutrients to damaged tissue
  • Accelerated nerve regeneration: PBMT supports axonal repair in damaged peripheral nerves, making it valuable for neuropathy and radiculopathy
  • Enhanced collagen synthesis: Fibroblasts are stimulated to produce new collagen, accelerating tendon, ligament, and disc repair

Class IV vs. Class III (Cold Laser)

The distinction matters clinically. Class III "cold" or "low-level" lasers operate below 500mW and cannot deliver therapeutic photon doses to deep tissue. For surface conditions like wound healing, cold laser may have some utility. For joint conditions, deep spinal pathology, or nerve pain — Class IV power levels are required.

Many practices advertise "laser therapy" without specifying the class. At McNamara Chiropractic Center, we use a true Class IV system.

How We Use Class IV Laser Therapy

Class IV laser is a core component of both our Knee Restoration Program and our Spinal Decompression Program. In combination with mechanical decompression (Antalgic-Trac or Knee-on-Trac) and shockwave therapy, laser accelerates the healing that decompression initiates.

Sessions are typically 5–15 minutes per treatment area. Patients wear protective eyewear during treatment. The treatment is painless — most patients feel a gentle warming sensation.
